** All School Physicals must be on the New Health Exam Form!!
**All students entering 6th grade will need to provide supporting documentation that they have received the tetanus booster (TDAP) as of September 20, 2021.**
**Students in Grades 7, 8, 9, 10 & 11 are required to have one dose of Meningitis Vaccine. Students in 12th grade are required to have two does of meningitis vaccine or 1 dose if first dose given after age 16.
**Please provide the school with supporting documentation that your child has received this immunization as of September 20, 2021.**

Health Appraisals
A physical examination, dated within 12 months prior, including Body Mass Index and Weight Status Category Information is required for all newly entering students and students in Pre-Kindergarten or Kindergarten, Grades 1, 3, 5, 7, 9, & 11.
*Our school physician recommends that your child receive their physical by his/ her own primary care physician.*
Dental Certificates
- A dental certificate is requested for all newly entering students and students in Kindergarten, Grades 1, 3, 5, 7, 9, & 11th.
Vision
- Distance acuity for all newly entering students and students in Kindergarten, Grades 1, 3, 5, 7, & 11th.
- Near vision acuity and color perception screening for all newly entering students and grades 1, 3, 5, 7, & 11th.

**A vision letter will be sent home to all student that receive a screening whether they pass or fail.**
Hearing
- Hearing screening for all newly entering students and students in Kindergarten, Grades 1, 3, 5, 7 & 11.
Scoliosis
- Scoliosis (spinal curvature) screening for (girls) grades 5 & 7 and for (boys) grades 5 & 9 .
A letter will be sent home for hearing and scoliosis screenings if there are any findings on the screening done at school that would cause concern or need medical follow-up. A vision letter will be sent home regardless of passing or failing the exam. Please call the school’s Health Office if you have any questions or concerns.
